Learn how to set up a form to collect information

Forms and surveys are a great way to capture additional information or create waivers for students or prospects. They can be added as a redirect to the profit generators or opt-in forms or be sent via email. Watch the video below for more information on how to create one. Using forms is more efficient as it saves the information directly to the lead's profile, compared to emailing waivers.

Additionally, waivers can also be accessed through kiosk apps, providing another convenient channel for students or prospects to complete and sign them.


Creating and Setting Up Waivers

To create a waiver in SPARK MEMBERSHIP:

  1. Navigate to the Forms & Surveys section from the left-side menu.

  2. Add your waiver content, including required fields and acknowledgments. You can copy and paste text from a previous system if needed.

  3. Save and publish the form. Optionally, set your checkout’s post-purchase redirect to the waiver’s URL so customers are directed to it immediately after purchase.

You can add a signature field to a form/survey and it will make it required for the student to enter.

To verify a signature was submitted, go to Forms & Surveys, locate the waiver, and open the specific submission via "View Submissions." Click "View Results and then Print" to display the full submission, including the captured signature.


Automating Waiver Processes

To ensure waivers are signed before appointments:

  1. Create appointment types in the calendar and add a tag for each type.

  2. Set up an automation triggered when a slot is booked with a specific tag.

  3. Configure the automation to send an email containing the waiver form to the student. This setup ensures students receive the waiver automatically after booking an appointment.


Accessing Waivers via the Kiosk App

To make waivers accessible through the kiosk app:

  1. Create a Home Screen Announcement in the member app.

  2. Paste the Forms URL into the announcement content.

  3. When tapped, the link opens the forms for leads to complete and sign.


Troubleshooting Common Issues

Preventing Youth Waivers from Overwriting Adult Accounts

If submitting a youth waiver overwrites an adult’s account:

  1. Enable the setting on the waiver form that distinguishes youth submissions from adult accounts.

  2. Check the designated checkbox to ensure the youth waiver links to the correct youth/guardian relationship.

  3. Save the form and re-test the submission to confirm the issue is resolved.
